Please call us today to schedule an appointment.  To help save you time, you may download and complete the forms and bring them with you to your first visit.  Payment of self-pay fees, co-payments, and outstanding balance payments are expected before being seen.​  Please read LLFHC Financial Policy for better understanding.  We accept cash, Visa, MasterCard, Discover and checks.​

  • Treatment of infectious processes of the body e.g respiratory, skin, with oral antibiotics.  IV antibiotics may be ordered
